Federal Regulations require that a Notice of Required Disclosures be provided to all enrolled and prospective students on an annual basis. Following is a list of the required disclosures.
Financial Assistance Information
This disclosure provides information on all available federal, state, local, private, and institutional financial need-based and non-need based assistance.
Information can be obtained at http://www.fresnostate.edu/studentaffairs/financialaid/ or please call the Financial Aid office at 559. 278.2182 or the Scholarship Office at 559. 278.6572. You may print your own copy from the website.
This disclosure provides general information related to: cost of attendance, procedures to withdraw from school, refund policies, return of Title IV funds, academic programs, accrediting agencies, special services for disabled students, and study abroad programs.

Completion/Graduation Rates for the Student Right-to-Know Act
This disclosure provides information on the number and percentage of students completing the program in which the student is enrolled or has expressed interest. Completion and Graduation Rates of Student Athletes
This disclosure provides information on the completion or graduation rate on student-athletes who received athletics aid and compares them to all undergraduate students who were enrolled in a full-time degree program at the same time. Graduation rates are based on a six-year enrollment period.
To access this information, go to http://www.ncaa.org/wps/wcm/connect/public/NCAA/Resources/Research/Graduation+Rates and click on Division I. Under "School Data", click on the letter "C" and a drop-down menu will pop up. Scroll down to "California State University, Fresno" and click on it. The graduation-rate report for the most recent six-year period will appear.

If you would like additional information about how the graduation rates are determined, go back to http://www.ncaa.org/wps/wcm/connect/public/NCAA/Resources/Research/Graduation+Rates and click on "Division I". Information about Division I schools will come up, and in the upper left-hand corner, click on "Information About the (Year) NCAA Graduation-Rates Report".
Equity in Athletics Disclosure Act (EADA)
This disclosure provides information on the number and gender of full-time undergraduates participating in the intercollegiate athletic competitions and on each team and the operating expenses of those teams. The report is called the Equity in Athletics Disclosure Act (EADA), and it is required of all colleges and universities receiving federal funding under Title IV and have intercollegiate athletic programs.
To find information on Fresno State participation rates and financial support, go to http://ope.ed.gov/athletics/GetOneInstitutionData.aspx. Under "State or outlying area" highlight "California", under "City", type in "Fresno", then under "Type of institution", highlight "Public, 4 year or above". Click on "Search" and "California State University, Fresno" should come up. Click on that and you will get information on Athletics Participation and Operating Expenses Per Team. If you scroll down to the bottom of that page and click on "Revenues and Expenses", you will get additional information on Revenues, Expenses and Athletically-Related Student Aid.
Campus Security Report (Jeanne Clery Act)
This disclosure provides comprehensive statistics on campus safety and crime, policies and procedures concerning the security of and access to campus facilities, policies concerning campus law enforcement, and campus safety and crime prevention programs (including drug, alcohol, and sex offense education programs). Information is available at http://www.fresnostate.edu/police/report.htm.
Drug and Alcohol Use Prevention
This disclosure provides information on the institution's compliance with Title 34 C.F.R. 668.41 that requires notice to all students, faculty and employees concerning its drug and alcohol abuse prevention program.

Family Educational Rights and Privacy Act (FERPA)
This disclosure informs students of their rights and the University's policies concerning access to and privacy of student educational records.

Teacher Preparation Programs
This disclosure provides data on all teacher preparation programs in the state. The California Commission on Teacher Credentialing (CCTC) compiles the report for California, including program information by institution. Specific information may be obtained from the Kremen School of Education and Human Development, Education Building, Room 100, 559.278.0300.
Use of Social Security Number
Title 5, California Code of Regulations Section 41201 (effective January 1971) authorizes CSU campuses to require an applicant for admission to "furnish the campus with his or her social security." Additional information can be obtained from the University Catalog.
